FACT Inspector Application Process

The following steps outline how to apply to become a FACT Inspector. You will be asked to gather specific documents and complete forms in electronic format, which will be uploaded within the Inspector Application.

Gather Documents

  • Current photo (head shot) (.jpg or .png)
  • Medical license and curriculum vitae or resume:
    • in one document
    • using one of the accepted file extensions: .doc, .docx, .xls, .xlsx, .ppt, .pptx, .pdf, .gif, .jpg, .jpeg, .png

Complete Application

  • After accessing the application using the button below, select "Login".  On the login page, you may enter your FACT Single Sign-On credentials.  If you do not have a pre-existing account, select "Create an Account".   If you need additional assistance, contact the FACT Helpdesk
  • Inspector Application Registration and Acknowledgement: Registration is for individuals who wish to apply to become a FACT Inspector.
  • Initial FACT Inspector Qualifications: Information on the qualification requirements to become a FACT inspector.
  • Complete Inspector Application: You will be required to upload a current photo (head shot) in order to submit your application.
  • FACT Conflict of Interest & Acknowledgements.
  • Submit Medical License and Curriculum Vitae or Resume. The following documents will need to be uploaded:
    • For inspectors who are physicians (MD, DO, MBBS), medical license verification or electronic copy of their current medical license.
    • Current curriculum vitae or resume in electronic format.
    • Submit both as one document.
  • This application will take approximately ten minutes of your time. Once you start the application, you must finish it. You cannot return to edit your inspector application.
